MANDATORY COVID-19 VISITATION PRECAUTIONS & GUIDELINES

[EFFECTIVE JANUARY 20, 2022] As the incidence of COVID-19 rises throughout our communities and region, visitation guidelines at Serenity House are adjusted to protect our patients and their families, our employees, and all those who rely on us for care. Therefore, in the spirit of health and safety, following are Serenity House visitation guidelines:

*** Visitation hours are limited to 8 AM to 8 PM. ***

*** The number of visitors are limited to 2 guests at a time per room. ***
One guest may stay overnight for patients who are actively at end-of-life.

1) All visitors must disclose VACCINATION STATUS … AND … All visitors must show documentation of a NEGATIVE COVID TEST – Antigen test within 1 day or a PCR test within 2 days.
     > “Fully Vaccinated” means individuals who are considered fully vaccinated for COVID-19: two weeks or more after they have received the second dose in a 2-dose series (Pfizer-BioNTech or Moderna or vaccine authorized by the World Health Organization), or two weeks or more after they have received a single-dose vaccine (Johnson and Johnson [J&J]/Janssen).

2) PRIOR TO ENTERING SERENITY HOUSE, all visitors are required to:
     > Check temperatures.
     > Clean their hands.
     > Complete COVID screening survey. Visitors with any symptoms will be restricted from entering.

    • Runny nose or sneezing
    • Coughing or sore throat
    • Shortness of breath
    • Pain with deep breathing
    • Pain with deep breathing
    • Chills
    • New loss of smell and/or taste
    • New GI issues like soft stool or stomach cramps

3) All visitors must WEAR a SURGICAL MASK or N95 MASK AT ALL TIMES – fully covering nose and mouth – regardless of vaccination status or COVID test result.
     > Fully Vaccinated and Boosted Visitors must wear a surgical mask.
     > Unvaccinated or Partially Vaccinated Visitors with a negative COVID test result must wear a N95 mask.
